Data Collection Form
The General Data Protection Regulations (GDPR) came into force in May 2018.
The Diocese has revised the "Database and Directory Return" form it has used in previous years in order to collect PCC contact information and replaced it with a Data Collection Form.
What we need from you
We are asking that the following PCC role holders complete and submit this form so that we can contact the correct people in your parishes for Diocesan business. This form will need to be completed only once, provided nothing changes.
In addition to updating the Diocesan Office with your details, it also provides an opportunity to give your consent for how we share you personal contact details. (i.e. whether you also allow us to add some details to the diocesan website).
If you start a new role, change your contact details, or wish to amend your consent preferences, simply complete and submit a new form and your details/preferences will be updated. We hope that most people choose (and are able) to complete this form online. But it is also available to download and return to us by post or email via the details below.
